Job Opportunity: Physical Therapist (PT) - Skilled Nursing Facility (SNF)

Location: Lancaster, California
Employment Type: Travel/Contract
Contract Length: 13 weeks (3 to 6 months assignment)

Job Details

Specialty: Therapy/Rehabilitation (Physical Therapy in SNF setting)
Shift: 5 days per week, 8-hour day shifts
Schedule Notes: May include occasional weekend coverage and floating as needed
Start Date: April

Experience Requirements

Minimum 1 year of experience in Skilled Nursing Facility (SNF) specialty preferred
New graduates will be considered
Prior travel experience is NOT explicitly required

Certifications & Licenses Required

Current State License verification (primary source online verification)
BLS (Basic Life Support) from American Heart Association or American Red Cross
ACLS (Advanced Cardiac Life Support) preferred
PALS (Pediatric Advanced Life Support) preferred
National Tech Certification as applicable (for tech roles)

Government issued photo ID (Driver’s License or State ID)
Emergency Contact Form prior to start
Background checks including:
- 7-year work/school/location history
- EPLS/GSA/SAM verification (annual)
- HHS/OIG verification (annual)
Competency exams as applicable by profession
Physical exam within 1 year of start date
Negative TB test or Chest X-Ray within 1 year (with specific rules for past positives)
Immunization or titer proof for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B
- Varicella (Chicken Pox)
9 Panel Drug Screen within 1 year of start
Seasonal Flu Vaccination or Declination during flu season (Oct 1 - Mar 31)
