Job Opportunity: Case Manager
Location: San Francisco, California
Employment Type: Travel/Contract
Contract Length: 13 weeks
Shift: Day (4 shifts of 10 hours each, 8:00 AM – 6:30 PM)
Hours per Week: 40
Job Description
- Work in a high-volume acute hospital environment.
- Responsibilities include discharge planning and patient care management.
- Must be self-directed, assertive, and creative in problem solving and systems planning.
- Ability to float to other facilities within the health system.
Required Qualifications
- Valid Registered Nurse (RN) license in California.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
- Minimum 2 years of case management experience, including experience within the last year.
- Experience in an acute care setting.
- Experience in discharge planning.
- Care manager experience.
- American Heart Association (AHA) Basic Life Support (BLS) certification.
