Job Opportunity: Case Manager

Location: San Francisco, California
Employment Type: Travel/Contract
Contract Length: 13 weeks
Shift: Day (4 shifts of 10 hours each, 8:00 AM – 6:30 PM)
Hours per Week: 40

Job Description

Work in a high-volume acute hospital environment.
Responsibilities include discharge planning and patient care management.
Must be self-directed, assertive, and creative in problem solving and systems planning.
Ability to float to other facilities within the health system.

Required Qualifications

Valid Registered Nurse (RN) license in California.
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
Minimum 2 years of case management experience, including experience within the last year.
Experience in an acute care setting.
Experience in discharge planning.
Care manager experience.
American Heart Association (AHA) Basic Life Support (BLS) certification.
