Job Opportunity: Speech-Language Pathologist (SLP) - Acute Care
Location: Kingman, Arizona
Employment Type: Travel/Contract
Contract Length: 20 weeks
Job Description
- Position in acute care setting.
- Flexible scheduling with consideration for time off.
Required Qualifications
- Valid Speech Language Pathologist license in Arizona
Required Documentation & Compliance
- Government-issued photo ID (Driver’s License or State ID).
- Background checks including:
- 7-year search within 30 days of start (updated annually).
- SS Trace, County searches for last 7 years of locations lived/worked/school.
- Nationwide Sex Offender check.
- Education verification (transcript/diploma or national verification).
- EPLS/GSA/SAM and HHS/OIG checks (annual).
- Copy of Transcript or Diploma required (or national verification if unavailable).
- Certifications as applicable (e.g., ACLS, BLS, PALS, NRP) depending on role requirements.
- Competency exams as applicable by profession/specialty.
- Clear, passport-style photo for badge.
- Facility-specific forms and agreements (security, confidentiality, behavioral expectations, HIPAA, etc.).
- Physical exam within 1 year of start date.
- Negative TB test or chest X-ray within 1 year of start.
- Annual TB questionnaire.
- Immunization or titer proof for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B
- Varicella (Chicken Pox)
- Negative 9-panel drug screen within 30 days of start.
- Seasonal flu vaccination or declination during flu season (Oct 1 - Mar 31).
- Two professional references within 3 years of start.
- Annual skills checklist.

- All required documentation and compliance must be maintained and updated annually as applicable.
- Physical and health documentation have specific expiration guidelines to follow.
- Facility-specific job descriptions may be required depending on degree or certification level.

Always floating, no home unit. Schedule can get crazy, don't know your unit until shift start usually. Would be rough for a 1st contract.
