Case Manager

Overview

Travel/Contract RN role in Santa Rosa, California.
Focuses on utilization management, resource management, discharge planning, post-acute care referrals, and care facilitation.
Supports acute patient populations across the care continuum, including discharge planning coordination and intervention for high-risk Emergency Department patients.
Shift: Day, 5x8 hours, 8:00 AM to 4:30 PM.
40 hours per week.
Contract length: 13 weeks.
If floated, works only in CM.

Required Qualifications

Valid RN license in California
Case Management (2 years of experience)
Case Management experience within the last year
BLS - Basic Life Support issued by American Heart Association or American Red Cross
Associate of Science in Nursing / Associate Degree in Nursing

Preferred Qualifications

Bachelor of Science in Nursing

Additional Notes

Minimum 2 weekend shifts per month.
