Job Description

  • Position at St. Joe Campus, 3600 E Harry
  • Travel assignment as a Registered Nurse in Medical/Surgical specialty

Required Credentials & Certifications

Licenses & Certifications

  • BLS (Basic Life Support) – Must be American Heart Association (AHA) certified
  • Nursys Verification – Within 30 days prior to start; disciplinary actions require FACIS Questionnaire
  • Current Nursing License – Primary source verification within 30 days prior to start
  • NIH Certification – Updated yearly (Group A yearly, Group B-F every 2 years)
  • MOAB Moderate Risk De-Escalation Training
  • Ascension Core Competencies – HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start, updated yearly, minimum 80% score
  • Ascension EKG Exam – Passing score of at least 80%, two attempts allowed
  • Ascension Cerner Test-Out – Cerner test results and training checklist with signed attestation
  • Ascension Skills Checklist – Clinical skills checklist completed within one year of start, updated annually

Personal Health History & Vaccinations

  • Ascension Color Vision Test – Completed prior to start, updated every 3 years
  • MMR Vaccination – Documented proof of two MMR vaccines or positive IGG titer; no declinations accepted
  • Hepatitis B – Documentation of vaccination series completion or signed declination
  • Influenza Vaccine – Required for flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement
  • Positive PPD or IGRA (TB Test) – Positive IGRA or PPD results, negative chest X-ray within a year, and annual TB questionnaire
  • 2 Step PPD or IGRA – Within 1 year and 30 days prior to start; annual updates required
  • Varicella – Proof of two vaccines, immunity, or history; no declinations accepted
  • Tdap – Valid within 10 years; no declinations accepted
  • Fit Test – Required for all travelers; preferred prior to start, updated annually
  • Ascension 12 Panel Drug Screen – Within 30 days prior to start, updated annually; includes 12 drug categories with MRO review
  • Health Statement – Statement of good health or physical exam prior to first assignment, updated annually

Professional Requirements

  • Driver's License/State ID – Must be current and match address requirements
  • Ascension Profile (Resume)
  • Two Professional References – Completed prior to first assignment
  • Criminal Background Check – Includes SSN trace, employment verification, OIG search, national sex offender check; updated annually or after break in service > 90 days
  • EPLS (Excluded Parties List System) Check – Within 90 days of start, rerun if break in service > 90 days
  • Education Verification – Verification of relevant education within 90 days of start; updated if break in service > 90 days
  • Employment Verification – Third-party verification for past 7 years; updated annually or after break in service > 90 days
  • Ascension Confirmation of Assignment – Provided upon initial assignment and updates with extensions

Onboarding & Training Requirements

  • Ascension Nursing Contract Worker Onboarding – Completed onsite prior to start
  • Ascension Onboarding Requirements Education Attestation – 6 hours pre-hire education with attestation
  • Ascension Nova StatStrip Training – Pre-hire modules for non-allied positions
  • Ascension Unit Specific Checklist – Optional for onboarding, must be uploaded within a week by unit leader

Optional / Additional Documents

  • Hiring Manager Evaluation – Post-assignment evaluation (optional)
  • Ascension Name Change Document – Legal documentation of name change (optional)
