Licenses & Certifications
- Nursys Verification: Must be completed within 30 days prior to start. If any disciplinary actions exist, complete FACIS Questionnaire.
- License/Certificate Verification: Current license and primary source verification within 30 days prior to start. FACIS Questionnaire required if disciplinary actions exist.
- BLS Certification: Must be American Heart Association (AHA) certified. Required before onboarding.
- NIH Certification: Group A updated yearly; Groups B-F updated every 2 years.
- MOAB Moderate Risk: MOAB De-Escalation Training required.
- Ascension EKG Exam: Must pass with at least 80%, max two attempts. Updated exam version required as of Dec 1, 2023.
- Ascension Cerner Test-Out: Cerner test results and training checklist with signed attestation required.
- Ascension Skills Checklist: Clinical skills checklist to be completed within one year of start and updated annually.
Professional Requirements
- Resume/Profile: Required for submission.
- Two Professional References: At least two qualifying references prior to first assignment.
- Criminal Background Check: Includes Social Security Trace, Employment Verification, OIG Search, National Sex Offender check, and more. Must be completed within 30 days of first assignment and updated annually or after breaks in service over 90 days.
- EPLS Check: Must be run within 90 days of start and rerun if break in service > 90 days.
- Education Verification: Must be completed within 90 days of start and rerun if break in service > 90 days. Official transcripts required if verification fails.
- Employment Verification: Third-party verification for past 7 years, updated annually or after breaks in service > 90 days.
- Onboarding Requirements Education Attestation: Completion of a 6-hour pre-hire module with attestation upload.
- Nursing Contract Worker Onboarding: Completed onsite.
- Ascension Nova StatStrip: Pre-hire modules required for non-allied positions before clearance.
- Ascension Confirmation of Assignment: Provided upon initial assignment and updated with extensions.
Personal Health History & Immunizations
- Ascension Color Vision Test: Completed prior to start and updated every 3 years.
- MMR: Documented proof of two vaccines or positive IGG titer. No declinations accepted. If negative, two doses four weeks apart required.
- Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider. No declinations accepted. If negative, two doses four weeks apart required.
- Tdap: Valid within 10 years. No declinations accepted.
- Hepatitis B: Documentation of vaccination series completion or signed declination.
- Influenza Vaccine: Required during flu season (Oct 1 - Mar 31). Exemptions accepted but mask required at all times.
- Fit Test: Required for all travelers, preferred before start but can be done onsite. Annual update required.
- 2 Step PPD or IGRA: 1st PPD within 1 year, 2nd within 30 days of start, or negative IGRA within 90 days. TB questionnaire updated annually. Positive TB requires chest X-ray within 1 year.
- Positive PPD - Negative Chest X-Ray and Annual TB Questionnaire: Optional but recommended.
- Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually.
- Ascension 12 Panel Drug Screen: Within 30 days prior to start and updated annually. Retesting required if break in service > 90 days. Includes testing for 12 specified substances with MRO review if applicable.
Identification
- Driver's License/State ID: Must be current, show front and back, and address must match technology records. For out-of-state placements, address must match home state for compact licenses.
- Ascension Badge Photo: Forward-facing headshot with white or blue background, no alterations or filters.
