Overview
- Role: RN Case Manager
- Specialty/setting: Case Management Discharge Planning
- Location: Boston, Massachusetts
- Contract length: 13 weeks
- Schedule: Day shift, Monday–Friday
- Hours: 40 hours per week; may be 4x10s or 5x8s depending on operational need
Required Qualifications
- Valid RN license in Massachusetts
- Associate of Science in Nursing / Associate Degree in Nursing
- 2 years of case management experience
- Case Management experience within the last year
- Experience in an Acute Care setting
- Discharge Planning
- Acute Hospital Case Management Experience
- License/certification verification required for submission
- Resume must include the past 7 years of work experience, with city and state for all work experience and education
- 2 references within 4 years of submission, including at least 1 supervisor or above in the skillset
- Skills checklist completed within 1 year of submission
- Candidate submission coversheet required for initial assignment and new facility submissions
Onboarding Requirements
- Fit test within 365 days of start date and updated annually
- Respirator medical clearance attestation required if fit test is required for the skill
- MMR documentation or proof of immunity
- Influenza vaccine required
- TB test documentation within 3 months of start date
- Health statement signed by MD/DO/NP/PA within 365 days of start date
- Criminal background check
- Massachusetts CORI
- Education verification
- SSN trace
- National sex offender check
- Exclusion checks
- Confirmation of assignment/POI form
- Beeline attestation

- Large organization/teaching hospital experience with 500 beds or more
