Overview
- Travel/Contract role in Green Bay, Wisconsin
- Start date: ASAP
- 8-hour day shifts
- This facility cannot accommodate 4x10s or 3x12s
- 13-week contract
- Home Health setting
- 1:1 ratio
- May visit up to 6 patients per day
- 30 productivity points standard
Required Qualifications
- Valid RN license in Wisconsin
- Associate of Science in Nursing / Associate Degree in Nursing
- Hospice (1 year of experience)
- Hospice experience within the last year
- One completed travel assignment
- BLS issued by American Heart Association or American Red Cross
- Driver's License
- Home Health experience
- Experience with Homecare Homebase
- Experience with OASIS
- Own transportation
- Proof of current auto insurance is required for all Home Health positions

Schedule / Logistics
- No floating between like units
- No block schedule
- Local candidates accepted within a 50–75 mile radius and paid at the same rate
- Occasional maximum radius of roughly 60 miles per day, with 60–90 minute total drive time
- Mileage reimbursement provided at IRS standard rate
Additional Notes
- Business casual attire; no jeans
- Scrubs allowed but not required
- Remote Client for Home Health and Rover are used on laptop and phone app, respectively
