Overview

• Travel/Contract role in Modesto, California
• Home health assignment
• 13-week contract
• Day shift: Monday–Friday, 8:00–17:00
• Guaranteed 40 hours per week
• Locals accepted at the same rate
• Primarily geriatric population
• Daily point expectation: 7–8 points
• Charting system: Perfect Notefield
• Phone interview

Required Qualifications

• Valid Occupational Therapist license in California
• ACLS issued by American Heart Association or American Red Cross
• BLS issued by American Heart Association or American Red Cross
• PALS issued by American Heart Association or American Red Cross
• State license verification required
• National tech certification verification required, as applicable
• Competency exam required as applicable by profession
• Emergency Contact Form required prior to start
• Government-issued photo ID required
• Background check required within 1 year of start
• EPLS/GSA/SAM required annually
• HHS/OIG required annually
• Physical exam within 1 year of start
• Negative TB test or chest X-ray within 1 year of start
• Hepatitis B documentation required
• Varicella documentation required
• Measles documentation required for candidates born in 1957 or later
• Mumps documentation required for candidates born in 1957 or later
• Rubella documentation required for candidates born in 1957 or later
• Drug screen within 1 year of start
• Flu vaccination or declination required during flu season
• 2 professional references within 3 years of start
• Skills checklist required annually
• Personal or auto insurance required for all home health positions

Preferred

• 1 year of PT experience
• ACLS
• PALS
• Perfect Notefield experience, as training will be provided
• Open to new grads
• Open to candidates without home health experience
