Physical Therapist - Outpatient (Travel/Contract)
Location: Vail, Arizona
Contract Length: 13 weeks
Shift: Day shift, 8:00 AM – 4:30 PM (5 days/week, 40 hours/week)
Job Summary
Provide physical therapy evaluation and treatment for outpatient physical therapy patients, including injured workers. Supervise physical therapy assistants and technicians, perform various functional and ergonomic evaluations, and maintain communication with referring physicians and rehab nurses. Participate in marketing and community outreach as needed.
Key Responsibilities
- Evaluate and treat patients per physician referral or direct access
- Supervise physical therapy assistants and technicians according to PT plan of care
- Perform Work Hardening Evaluations, Functional Capacity Evaluations, ergonomic evaluations, and post-offer testing (e.g., WorkSTEPS, APET)
- Utilize modalities and exercise equipment available in the facility
- Manage scheduling, referrals, and phone communications as needed
- Maintain accurate billing code usage for various payer types
- Provide weekly patient progress updates to rehab nurses and timely notes to referring physicians
- Maintain professional relationships with patients, physicians, case managers, and other contacts
- Assist with business development and marketing efforts when patient load allows
- Participate in community events representing the facility
- Perform additional tasks as required for operational success
Required Qualifications
- Valid Physical Therapist license in Arizona
- Minimum 1 year of Physical Therapist experience within the last year
- Experience in an outpatient setting
Compliance & Documentation Requirements
- Government-issued photo ID (Driver’s License or State ID)
- Background checks including 7-year residence/work history, sex offender registry, and federal exclusions (updated annually)
- Copy of diploma/transcript
- Physical exam within 1 year of start date
- Negative TB test or chest X-ray within 1 year (with specific protocols for past positive results)
- Immunization or titer proof for Measles, Mumps, Rubella, Hepatitis B, Varicella (per birth year and policy)
- 9-panel drug screen within 1 year of start date
- Seasonal flu vaccination or declination during flu season (Oct 1 – Mar 31)
- Annual competency exams and skills checklist as applicable
- Emergency contact form prior to start
- Employment application/resume
- Professional references:
- 2 for travel, allied, or per diem acute care roles
- 1 for per diem non-acute care roles

- Maintain up-to-date employee information for liability insurance coverage
- Demonstrate professionalism with all contacts and stakeholders
- Willingness to perform duties beyond the job description for team success
