Valid RN license in Texas
Rehabilitation (1 year of experience)
Rehabilitation experience within the last year
ACLS - Advanced Cardiac Life Support issued by American Heart Association or RQI or American Red Cross or American Safety and Health Institute
BLS - Basic Life Support issued by American Heart Association or American Red Cross or RQI or American Safety and Health Institute
NIHSS
CPI - Crisis Prevention Institute
Associate of Science in Nursing / Associate Degree in Nursing

Physical exam within 12 months of start
Fit test within 1 year of start
Tetanus/diphtheria/Tdap vaccination within 10 years of start
Negative TB test or chest X-ray within 1 year of start
TB questionnaire required for positive TB results
Measles, mumps, rubella, hepatitis B, and varicella immunity/vaccination documentation required
Negative 10-panel drug screen within 90 days of start
Flu vaccination or approved exemption required during flu season
Color blind testing required
