Work Schedule
- Day shift, Monday through Friday
- 8 hours per day (8:00 AM – 5:00 PM)
- Weekend work may be required depending on census needs but schedule will be adjusted to maintain a 40-hour workweek
- On-call may be needed occasionally but not regularly
Job Description
- Provide and coordinate home nursing care according to physician orders and agency policies
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses
- Manage an average caseload of 5-8 patients per day, including start of care, wound care, and follow-up visits
- Must be willing to travel up to 60-75 miles radius from home clinic, averaging approximately 200 miles traveled per week
- Documentation and charting using HomeCare HomeBase (HCHB) and OASIS systems; proficiency required within the last 6 months or retraining provided
Required Qualifications
- Valid RN license in Oregon
- Minimum 1 year of hospice experience within the last year
- Experience in a Home Health setting
- BLS certification
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Proficiency with HomeCare HomeBase documentation system
Additional Information
- First-time travelers are not accepted; recent specialty experience is mandatory
- Local candidates accepted within the same pay structure
- Dress code: Scrubs
- Productivity expectations: 25-30 units per week including admissions
- The position is due to increased census from a hospice closure in the area
Notes
- Mileage reimbursement follows IRS standard rate for travel between patient homes; commute to/from first and last patient is not billable
- Some positions may follow a Baylor shift schedule (4x10s, Friday-Monday), but most are standard 5x8 schedules
- Weekend and on-call requirements vary and should be discussed during the interview process
