Registered Nurse (RN) – Community Health (Days)

Job Summary

Provide, teach, supervise, and document nursing care for patients in their homes. Promote, maintain, or restore health, prevent illness, and support rehabilitation. Collaborate with healthcare providers and community representatives to coordinate home care. Evaluate patient and family responses to interventions. Function independently under general guidance, following unit-specific policies and protocols.

Key Responsibilities

• Deliver nursing care to sick and injured patients at home.
• Use critical thinking and independent judgment in clinical decision-making.
• Manage time effectively and communicate clearly, both verbally and in writing.
• Supervise others and maintain confidentiality of patient information.
• Maintain accurate clinical records.
• Be flexible with work hours and accept on-call responsibilities.
• Drive in inclement weather as needed.
• Adhere to customer service, patient rights, and performance standards.
• Submit any requested time off (RTO) at the time of application; no more than 5 days allowed.

Required Qualifications

• Valid RN license in Massachusetts
• Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN)
• Minimum 1 year of acute nursing care experience, hospital-based
• Minimum 1 year of home health nursing experience within the last year
• Certified Home Health Nurse with proficiency in OASIS
• Active BLS certification issued by the American Heart Association
• Experience in supervisory roles
• Current driver’s license and insurance; own vehicle required

Preferred Qualifications

• Infusion Therapy Certification.

Contract Length

• Approximately 13 weeks.

• Candidates residing within 50 miles of the facility will not be considered.
• Must provide at least two specific interview dates/times when applying; vague availability (e.g., "anytime") is not accepted.
• All scheduling requests, including time off, must be submitted with the application; requests after submission or during interview will not be approved and may result in offer rescission.

  • Work history
  • Skills checklist for Home Health RN (current within 1 year)
  • One supervisory signed reference from the past two years
  • Massachusetts RN license
  • Current BLS certification

Important Compliance

• Must hold a Massachusetts Temporary Nursing Service Agency License to be eligible for this assignment.
• Must conform to all applicable policies and standards related to patient care and confidentiality.
