<h2>Job Description</h2>
<p>The Community Health LPN is responsible for delivering and documenting nursing care to patients in the home environment. This role is essential in promoting, maintaining, and restoring health to prevent illness and facilitate rehabilitation.</p>

<h2>Key Responsibilities</h2>
<ul>
    <li>Provide nursing interventions to promote patient health and prevent illness.</li>
    <li>Collaborate with healthcare providers and community representatives to coordinate home care.</li>
    <li>Evaluate individual and family responses to nursing interventions.</li>
    <li>Function independently under general guidance from the Director, adhering to specific guidelines and protocols.</li>
    <li>Utilize effective time management, communication, planning, and problem-solving skills in care provision.</li>
    <li>Exhibit critical thinking and maintain emotional stability in emergency settings.</li>
</ul>

<h2>Qualifications</h2>
<ul>
    <li>Ability to exercise independent judgment.</li>
    <li>Demonstrated interpersonal skills.</li>
    <li>Maintain confidentiality of patient information.</li>
    <li>Develop and maintain clinical records and communicate data in both verbal and written forms.</li>
    <li>Flexible with work hours and willing to drive in inclement weather.</li>
    <li>History of dependability in meeting work schedule commitments.</li>
    <li>Must possess a current driver’s license and insurance.</li>
    <li>Experience in home health care is required.</li>
    <li>Experience with self-scheduling of caseload is preferred.</li>
    <li>Exposure to community health principles and nursing practices.</li>

<h2>Orientation and Time Off</h2>
<p>Non-billable orientation will require 8 hours. Requested time off during the assignment must be noted and verified prior to uploading the profile. Only up to five days of requested time off will be accepted. It is essential for candidates to be flexible regarding holiday schedules, particularly during major holidays.</p>

<h2>Certifications Required</h2>
<ul>
    <li>Previous experience in acute nursing care preferred.</li>
    <li>Current Massachusetts LPN license.</li>
