Outpatient Hospice / Medical Social Worker
Location
- Albany, Oregon
Contract
- 13 weeks
- Contract role
- 24 hours per week
- Day shift: 0800-1630
- No weekends
Setting
- Hospice services provided in home-based settings, including private residences, skilled nursing facilities, assisted living facilities, and adult foster homes
Required Qualifications
- Valid Licensed Clinical Social Worker license in Oregon
- Licensed Clinical Social Worker (1 year of experience)
- Licensed Clinical Social Worker experience within the last year
- BLS
- Driver's License
- LMSW - Licensed Master Social Worker experience
Preferred Qualifications
- Previous hospice experience
- Oregon LCSW
- Experience as a social worker in a hospital or medical setting
Role Notes
- Provides hospice support services for patients and families, including skilled nursing coordination, pain and symptom management, medical social services, counseling, spiritual support, hospice aide support, volunteer support, and bereavement support
- Medications and equipment are included as appropriate
