Minimum Requirements 
<ul><li>Master's or Doctoral Degree in Physical Therapy from an accredited program.</li><li>Active Texas Physical Therapist license required prior to start.</li><li>BLS certification from AHA or ARC may be required.</li><li>Home health and HCHB experience required.</li><li>Experience with home health documentation: SOCs, ROCs, recertifications/reassessments, and plan of care development.</li><li>Experience supervising PTAs.</li><li>Preferred: orthopedics and neuro experience.</li></ul>

<ul><li>Facility Type: Home Health.</li><li>Shift Name: Days.</li><li>Shift Details: Monday–Friday, 08:00–16:00.</li><li>Weekly Hours: 40.</li><li>Productivity Expectation: 30 points.</li><li>Coverage Areas: Hawkins, Mineola, Lindale (local travel between patient visits required). Mileage reimbursed between visits from first to last patient.</li><li>Primary responsibilities: patient assessment, complete plan of care for home health, SOCs, ROCs, recerts/reassessments, supervise PTA.</li><li>Dress code: scrubs in black, gray, or purple.</li></ul>
