Job Overview
- Provide nursing care under direct supervision of a Registered Nurse or physician.
- Perform nursing duties on an assigned unit, using tact and judgment in patient care.
- Assist in supervising non-professional staff as needed.
- Follow nursing department philosophy, standards, objectives, and policies.
Responsibilities
- Deliver proficient nursing care based on knowledge of illnesses, injuries, and diseases.
- Use diagnostic results and other information to evaluate patient condition accurately.
- Perform timely, comprehensive, and accurate patient reevaluations and documentation.
- Maintain patient privacy during evaluations.
- Contribute to individualized patient care plans with measurable goals.
- Adjust care plans as patient conditions change and monitor outcomes.
- Coordinate discharge planning and post-institutional care needs.
- Collaborate with Care Managers, physicians, and other clinical staff to plan interventions.
Required Qualifications
- Valid LPN/LVN license in New York.
- Minimum of 1 year of LPN/LVN experience within the last year.
- Experience in a Long Term Care (LTC) or nursing home setting.
Work Schedule
- 5 days per week, 8-hour shifts.
- Shifts available: Days (7:00 AM – 3:15 PM), Evenings (3:00 PM – 11:15 PM), Nights (11:00 PM – 7:15 AM), all with every other weekend off.
- Weekend-only roles also available.
Skills and Physical Requirements
- Ability to work in environments with dust, noise, and potential exposure to mechanical, electrical, radiant, explosive, burn, and chemical hazards.
- Ability to hear, talk, walk, stand, bend, stretch, and sit for extended periods.
- Ability to read, write, and interpret documents.
- Ability to communicate effectively with patients and coworkers.
- May be required to handle hazardous waste/materials and operate/clean equipment.
- May assist in lifting patients onto/out of exam beds or procedure stations.
- Must be knowledgeable and trained in policies related to hazardous materials and waste.
- Routine potential exposure to blood, body fluids, and tissue.
Additional Information
- Experience with EPIC electronic health records system is a plus.
- Strong professionalism required.
