Home Health/Hospice RN

Location

• Iron Mountain, Michigan

Contract

• 13 weeks
• 40 hours per week
• Day shift, 8 hours per day

Required Qualifications

• Registered Nurse license
• BLS certification
• Recent home health experience within the last 2 years
• OASIS documentation proficiency
• Wound care experience
• Excellent infection control practices in the home environment

Schedule

• Monday through Friday, 0800–1630
• Rotating weekends
• Rotating holidays
• Evening on-call rotation
• One weekend on call per month
• One evening on call per week

Patient Population / Case Mix

• Home health and hospice patients
• Common needs include CHF, diabetes, renal disease, wound care, IV therapy, and management of drains and other devices

Responsibilities

• Provide home health and hospice care
• Complete SOCs, recerts, and resumptions
• Participate in on-call coverage

Additional Notes

• All clinicians must clock in and out at the facility
• Candidates living within 80 miles of the facility are considered local
• If previously worked there, the clinician must have an end date at least 6 months before submission
