Role Overview

• Inpatient Pharmacist
• Overnight schedule: Mon-Fri 9pm-7am; Sat/Sun 8pm-7am
• 7-on, 7-off

Required Qualifications

• Valid Pharmacist license in New York
• Pharmacist with 4 years of experience
• Pharmacist experience within the last year
• ACLS
• Driver's License
• Experience with Epic

Onboarding / Compliance Requirements

• Influenza vaccine or specific declination form required; if declination is used, mask must be worn at all times on assignment
• MMR immunity or 2-dose vaccine series required
• Varicella immunity or 2-dose vaccine series required
• Hepatitis B immunity, 3-dose vaccine series, or specific declination form required
• Tdap within the last 10 years, or specific declination form if not on file
• TB testing required
• Fit test required prior to start and within the last 12 months
• Physical/health statement within the past 12 months
• COVID-19 vaccine proof or declination is optional
• Valid/current US driver’s license or US ID card required
