Schedule

• 7:00 AM–3:30 PM
• 5 shifts per week
• Occasional 9:00 AM–5:30 PM shift
• Weeknight and weekend call

Contract Length

13 weeks

Required Qualifications

• Valid RN license in Ohio
• Associate of Science in Nursing / Associate Degree in Nursing
• Driver's License
• Operating Room (1 year of experience)
• Operating Room experience within the last year

Onboarding / Compliance Requirements

• 2-step TB test or IGRA (QuantiFERON/T-Spot) with negative reads within the last 6 months; annual requirement
• If TB test is positive: clear chest X-ray and current TB questionnaire required
• Influenza vaccine proof required October through March
• If declining flu vaccine: masking attestation form and influenza vaccine declination form required
• If medically exempt from flu vaccine: influenza vaccine medical accommodation form and physician-signed letter on medical office letterhead required
• Background check within 6 months of start
• OIG search within 6 months prior to start
• SAM/EPLS/GSA/OFAC searches within 6 months prior to start
• Drug screen within 60 days of start
• Safety and compliance training modules must be completed before clearance
• Badge photo required
• Submission packet required, including resume, job application, and prior employment form

Notes

• Chest X-rays are valid for 5 years
• If there is a break in assignment greater than 60 days, a new drug screen is required
• If there is a break in assignment of 6 months or more, a new background check is required
