• COVID-19 Vaccine (Proof of vaccination required)
• MMR Vaccine (Proof of vaccination required)
• Hepatitis B Vaccine (Proof of vaccination required)
• Influenza Vaccine (Proof of vaccination required)
• Varicella Vaccine (Proof of vaccination required)
• Tdap Vaccine (Proof of vaccination required)
• Physical/Health Statement (Documentation of fitness to work)
• Trinity 9 Panel Drug Screen (Completed within 30 days of start)
• Fit Test (Required for Onboarding)
• Trinity Specialty Specific Competency Testing (Attach copy of specialty exam)
