Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) - Travel Contract

Location: Clifton Springs, New York
Employment Type: Travel/Contract
Contract Length: 13 weeks
Shift: Night shift, 5 days per week, 8-hour shifts (7:00 PM - 7:30 AM)
Weekend Requirement: Every other weekend
Holiday Requirement: Available to work every other holiday, if needed

Job Description

• Work in a nursing facility/nursing home setting
• Patient ratio approximately 1:10
• Floating within skillset may be required
• Charting experience with EPIC system required
• Accepting first-time travel nurses
• Locals accepted with adjusted pay

Required Qualifications

• Valid LPN / LVN license in New York (NY is not a compact state)
• Minimum of 1 year LPN / LVN experience, with experience within the last year
• Experience in a Long Term Care setting
• Basic Life Support (BLS) certification issued by the American Heart Association
• Proficiency in behavior management techniques
• Experience with EPIC charting system

Scheduling and Policies

• Must submit requests for time off, block, and special scheduling upon submission
• Client may cancel up to three shifts during the 13-week contract without penalty

Required Documentation and Compliance

• Government-issued photo ID (Driver’s License or State ID)
• 7-year criminal background check including SSN trace, national criminal, sex offender, and education verification
• EPLS/GSA/SAM and HHS/OIG checks within 3 months of start, updated monthly
• Foreign education equivalency evaluation if applicable
• Education verification from primary source (not required for CNA)
• Primary source verification of state license within 30 days of start
• NURSYS verification for LPN/LVN within 30 days of start
• Photo for badge (passport-style with solid background)
• Physical exam within 1 year of start, updated annually
• Fit mask test within 1 year, updated annually (specific mask types accepted)
• Negative TB test or chest X-ray within 1 year, with annual TB questionnaire if applicable
• Immunizations or positive titers required for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (surface antibody positive titer, completed series, or declination)
  • Varicella
• Drug screening within 30 days of start (amphetamines, cocaine, fentanyl, heroin, opiates, etc.)
• Seasonal flu vaccination or signed declination during flu season
• Snellen eye exam within 1 year of start
• COVID vaccination copy preferred but not required

Additional Requirements

• References: 2 references within 3 years (required for travel agency submission)
• Employment application and skills checklist annually
• Work history verification of at least 2 past employments
• New York State Nurse Aide Registry registration required within 3 months for LTC and related units
• Fingerprinting and additional background checks may be required for certain units (Mental Health, Behavioral Health, Home Health Aide, LTC)

Certifications (As Required by Unit)

• ACLS, PALS, NRP, TNCC, EFM certifications may be required depending on unit assignment
• Medication/pharmacology competency exams and other competency exams as required

Uniform

• Scrub colors: Any combination of navy blue and white

Note: Prior travel experience is not required for this position.
