Job Opportunity: Therapy/Rehabilitation - Physical Therapist (Home Health)
Location: Gainesville, Georgia

Job Description
- Provide Physical Therapy services in a Home Health setting with home visits and periodic office meetings.
- Average patients per day: 5-6
- Patient load: 5-10 patients per day, equating to roughly 6-8 points daily (points system based on visit type and activities).
- Types of cases: Start of Care, Follow Ups
- Schedule: Monday to Friday, 8 AM to 5 PM (some positions may have alternate schedules such as 4x10 shifts Friday-Monday)
- On-call: Yes, every 4th weekend
- Travel: Must be willing to travel up to 75 miles from home clinic; daily travel expected up to 50-60 miles
- Productivity expectation: Minimum 30 points per week
- EMR systems used: HomeCare HomeBase, Oasis, Outlook
- Patient visits: Only in patients' homes (no facility visits)
- Locals accepted at the same rate
- Guaranteed hours: No, but floating to nearby clinics possible if patient census is low
Requirements
- Specialty: Physical Therapy in Home Health
- Minimum 1 full year of recent Adult Home Health experience (recent experience required; first-timers not accepted)
- Certifications: CPR and BLS (current)
- Must have reliable transportation and a current driver's license
- Must have agency-required liability insurance
- License: Must have license in hand or be able to obtain within a few days
- Willingness to discuss weekend and on-call requirements during interview

- References: Two references required for all placements
- Work history verification: At least two most recent employers or last 7 years if only one employer
- Various facility-specific compliance documents and training must be reviewed and signed prior to start (e.g., HIPAA, Code of Conduct, Compliance Training, Privacy Policy, Home Health Scenarios, etc.)
