Case Manager

Overview

  • Travel/Contract role in Mission Hills, California
  • Start date: ASAP
  • Day shift, 5x8 hours, 8:00–16:30
  • 40 hours per week
  • 26-week contract
  • No floating expectation
  • Patient ratio: 1:20
  • Weekend rotation required to meet department needs
  • First-time travelers are accepted and will be screened by clinical
  • RTO limit: no more than 7 days

Required Qualifications

  • Valid RN license in California
  • Associate of Science in Nursing / Associate Degree in Nursing
  • Case Management (2 years of experience)
  • Case Management experience within the last year
  • Experience in an Acute Care setting
  • Acute Hospital Case Management Experience
  • Discharge Planning
