Case Manager

Overview

• Travel/Contract RN role in Santa Rosa, California.
• 13-week assignment.
• Day shift: 5x8s, 0830-1700.
• Minimum 2 weekend shifts per month.
• If floated, works only in CM.

Role Summary

• Coordinates utilization management, resource management, discharge planning, post-acute care referrals, and care facilitation.
• Oversees acute patient populations across the care continuum to help prevent unnecessary admissions or readmissions.
• Provides discharge planning coordination and intervention for high-risk patients presenting in the Emergency Department.

Required Qualifications

• Valid RN license in California
• Case Management experience within the last year
• 2 years of Case Management experience
• BLS - Basic Life Support issued by American Heart Association or American Red Cross
• Associate of Science in Nursing / Associate Degree in Nursing

Preferred Qualifications

• Bachelor of Science in Nursing

• 7-year background check
• EPLS/GSA/SAM
• HHS/OIG
• FACIS Level 3 screening
• California Medi-Cal search result if FACIS III cannot be run monthly
• Sex offender search
• Employment application with full 7-year work history and no gaps
• Unit-specific skills checklist

• Physical exam
• Fit mask test
• TB screening and negative TB or chest X-ray documentation
• Drug screen
• Flu vaccination documentation during flu season
• Color blind exam
