Overview
- Travel/Contract role in Atlanta, Georgia
- Day shift: Monday-Friday, 8:00-16:30
- 40 hours per week
- Floating within facility/scope of practice as needed
- Locals accepted
Required Qualifications
- Valid RN license in Georgia
- Associate of Science in Nursing / Associate Degree in Nursing
- 3 years of Case Management experience
- Case Management experience within the last year
- One completed travel assignment
- Full compliance required to start on time
- Georgia license verification is required for CRTs and RRTs only
- Required by Aya: 2 clinical references, skills checklist within 12 months, verification of most recent employer, physical exam within 12 months of start, color blind testing, competency exam, medication test/pharmacology competency exam, and annual Nursys verification if applicable
Role Details
- Assess, evaluate, and drive discharge planning
- Coordinate services to the patient’s appropriate level of care
- Initiate, evaluate, and re-evaluate the treatment plan daily
- Monitor progress and barriers to discharge
- Monitor length of stay
- Follow Medicare regulatory guidelines regarding patient choice, IMM notices, and MOONS
Schedule and Time Off
- No weekends
- No call
- Maximum 7 days of requested time off
- No requested time off during September
- Holiday coverage is expected based on how many major holidays fall during the assignment
Compliance and Health Requirements
- Background check must be complete before start
- Negative 10-panel drug screen within 60 days of start
- TB testing or chest x-ray required within 12 months of start
- TB questionnaire required for all candidates
- MMR, varicella, and hepatitis B documentation are required
- Flu vaccine or declination is required
