Valid RN license in California
Home Health (1 year of experience)
Home Health experience within the last year
1 year or more of total travel experience
BLS issued by American Heart Association
Bachelor of Science in Nursing
Experience in an Acute Care setting
Driver's License
Experience with Epic

Job Summary

Provides case finding, consultation, health-focused assessment, care coordination, and care planning for adults with disabilities, older adults, and their families in home and community-based settings.
