Required Credentials & Compliance for Onboarding

Licenses & Certifications

• Current Nursing License: Must have primary source verification within 30 days prior to start.
• Nursys Verification: Required within 30 days prior to start.
• BLS Certification: Must be American Heart Association (AHA) certified.
• NIH Certification: Group A updated yearly; Groups B-F updated every 2 years.

Exams & Testing

• Ascension Medical Surgical Exam: Minimum 80% score, max 2 attempts, updated every 3 years.
• Ascension Core Competencies: Includes HIPAA, Joint Commission NPSG, OSHA topics; minimum 80% score, max 2 attempts, completed within a year of start and updated yearly.
• Ascension EKG Exam: Passing score of at least 80%, max 2 attempts; must be current edition as of Dec 1, 2023.
• Ascension Cerner Test-Out: Cerner test results and training checklist with signed attestation required.
• Ascension Skills Checklist: Clinical skills checklist to be completed within one year of start and updated annually.

Health & Immunizations

• MMR: Proof of two vaccines or positive IGG titer; no declinations accepted.
• Varicella: Proof of two vaccines, serological immunity, or documented history; no declinations accepted.
• Tdap: Valid within 10 years; no declinations accepted.
• Hepatitis B: Documentation of vaccination series completion or signed declination.
• Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); declinations accepted with mask requirement.
• Fit Test: Required for all travelers; preferred before start, can be done onsite.
• 2 Step PPD or IGRA: First PPD within 1 year, second within 30 days of start; or negative IGRA within 90 days. TB questionnaire updated annually.
• Positive PPD: Requires negative chest X-ray within 1 year and annual TB questionnaire.
• Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually.
• 12 Panel Drug Screen: Within 30 days prior to start and updated annually; includes testing for 12 specified substances.

Professional Requirements

• Resume/Profile: Must be submitted prior to assignment.
• Two Professional References: Required before first assignment.
• Criminal Background Check: Includes SSN trace, employment verification, OIG search, national sex offender check; updated annually or after break in service >90 days.
• EPLS Check: Must be run within 90 days of start and rerun if break in service >90 days.
• Education Verification: Relevant education verified within 90 days of start; must be rerun if break in service >90 days.
• Employment Verification: Past 7 years verified; updated annually or after break in service >90 days.
• Confirmation of Assignment: Provided upon initial assignment and updates with extensions.

Onboarding & Orientation

• Ascension Nursing Contract Worker Onboarding: Completed onsite.
• Ascension Onboarding Requirements Education Attestation: Completion of 6-hour pre-hire training with attestation.
• Ascension Nova StatStrip: Pre-hire modules required for non-allied positions.
• Ascension Badge Photo: Forward-facing headshot with white or blue background, no alterations.

Additional Notes

• Flu season mask policy applies if influenza vaccine is declined.
• Some optional documents and surveys may be requested post-assignment or during onboarding.
• All required documents must be current and compliant with Ascension Health policies.

This position is ideal for experienced Medical/Surgical RNs who meet the above credentialing and compliance requirements and are available to start on December 8 for a 13-week assignment in Nashville, TN.
