Physical Therapist - Home Health (Travel/Contract)

Location: Douglas, Georgia
Contract Length: 13 weeks
Schedule: Monday to Friday, 8:00 AM – 5:00 PM (40 hours/week)
Shift: Day (5x8 hours)

Job Description

• Provide physical therapy services during home visits.
• Average patient load: 5-6 patients per day.
• Types of visits: Start of Care and Follow-Ups.
• Must have reliable transportation and a current driver’s license.
• Willingness to travel up to 75 miles from home clinic; typical travel 30-40 miles per day.
• Use of EMR systems: HomeCare HomeBase, Oasis, Outlook.
• No on-call required.
• Weekend requirements vary; discuss during interview.
• Local candidates accepted at the same rate.
• Contract may lead to permanent hire if candidate is willing.

Required Qualifications

• Valid Physical Therapist license in Georgia.
• Minimum 1 year of recent Physical Therapist experience within the last year.
• Experience working with adult patients in a Home Health setting.
• Current BLS certification.

Productivity Expectations

• Weekly productivity units: 25-30.
• Points system used to measure productivity (approx. 6-8 points daily).

Compliance and Documentation Requirements

• Must complete and maintain all required background checks, including:

  • 7-year standard background check with SS trace, county, nationwide, sex offender, and state-specific searches.
  • EPLS/GSA/SAM and HHS/OIG checks within 14 days of start, updated monthly.
  • Fingerprinting as required by state (varies by location).
• Education verification for degree applicable to Physical Therapy.
• Two professional references required.
• Work history verification for at least two most recent employers or last 7 years.
• Must complete all facility-specific documents and compliance training prior to start, including but not limited to:

  • Code of Conduct, Confidentiality Forms, HIPAA and Privacy Policy, Compliance Training Certification, COVID-19 Policy and Attestation, Home Health Scenarios, and others as applicable.
• Physical exam within 12 months prior to start confirming good health and no communicable diseases.
• Negative TB test or chest X-ray within 12 months, updated annually.
• Immunization or titer proof for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella as per standard requirements.
• 10-panel drug screen required.
• Seasonal flu vaccination or documented declination required.
• COVID-19 complete vaccination series or declination required.

Additional Notes

• Schedule may vary in some territories (e.g., 4x10 shifts Friday-Monday possible, but most are standard 5x8).
• Mileage reimbursed at IRS standard rate for travel between patient homes; commute to/from first and last patient is not billable.
• On-call and weekend requirements vary by location and should be discussed during interview.

This position is ideal for experienced Physical Therapists with recent adult home health experience who are comfortable with travel and working independently in patients’ homes.
