Job Opportunity: Physical Therapist (PT) - Home Health

Location: Oakland, California
Employment Type: Contract
Contract Length: 13 weeks

Role Overview

Prioritize start of care visits
Supervise Physical Therapy Assistants
Mandatory next-day reporting of start of care visits
Complete clinical notes and documentation within 48 hours of visit
Maintain timely communication with supervisor, intake, and office
Be generally responsive during normal business hours

Schedule

40 hours per week, Monday to Friday
One weekend per month rotation

Training

3 full days (1 in-house, 2 in the field)

30 points per week (1 point = 1.33 hours)
Caseload averages 8 hours per day, 40 hours per week

Revisit, Non-OASIS Discharge: 1 point

Required Qualifications

California Physical Therapist (PT) License
Minimum 1 year of Home Health experience

Territory includes Sacramento, CA, with possible expansion
Mileage, parking, and toll reimbursement provided
Company car option available (with adjusted pay)
