Community Hospice Primary RN

Grand Junction, Colorado

Contract (12 weeks)

Monday to Friday, 8:00 AM – 5:00 PM
Occasional weekend backup and on-call duties

40 hours

Provide compassionate hospice nursing care in patients' homes, nursing homes, group homes, or assisted living facilities.
Serve as a primary RN with an assigned caseload, leading the interdisciplinary team for those patients.
Act as a generalist in all diseases and a specialist in symptom management, requiring strong assessment and critical thinking skills.
Educate patients and caregivers to help identify personal goals of care and build caregiver confidence.
Work in an environment that fosters personalized, life-changing care.

Valid RN license in Colorado
Minimum 2 years of hospice nursing experience, including experience within the last year
Associate of Science in Nursing or Associate Degree in Nursing
