Physical Therapist - Home Health

Location: Oakland, California (Solano County Territory, with possible expansion)
Employment Type: Contract
Contract Length: 13 weeks

Role Responsibilities

• Prioritize start of care visits
• Supervise Physical Therapy Assistants
• Mandatory next-day reporting of start of care visits
• Complete clinical notes and documentation within 48 hours of visit
• Maintain timely communication with supervisor, intake, and office
• Be generally responsive during normal business hours

Schedule

• 40 hours per week
• Workdays: Saturday through Wednesday
• Training: 3 full days (1 in-house, 2 in the field)

Documentation & Equipment

• Documentation system: Kinnser
• Equipment provided: iPad, Verizon One Talk

Productivity

• Requirement: 30 points per week
• Point to hourly conversion: 1 point = 1.33 hours
• Caseload and visit mix average 8 hours per day, 40 hours per week
• Overtime requires pre-approval by Clinical Supervisor

Visit Types & Point Values

• Start of Care: 2 points
• Recertification, Resumption of Care, OASIS Discharge, Evaluation: 1.5 points
• Revisit, Non-OASIS Discharge: 1 point

Required Qualifications

• Valid Physical Therapist license in California
• Minimum 1 year of Physical Therapist experience within the last year
• Minimum 1 year of experience in a Home Health setting
• Experience in supervisory roles

Additional Notes

• Mileage reimbursement and parking/toll reimbursement provided
• Company car option available (may affect pay structure)
