Hospice Registered Nurse - Vermont, Rutland

Job Type

  • Contract
  • Duration: 13 weeks
  • Hours: 37 hours per week
  • Shift: Tuesday - Saturday, 8am-4pm
  • Some on-call required

Job Description

  • Conduct comprehensive assessments of patient and caregiver needs using the nursing process.
  • Develop and communicate realistic, individualized plans of care involving patients and caregivers.
  • Participate actively in interdisciplinary team (IDT) meetings to develop and update care plans.
  • Serve as Case Manager for assigned patients, managing caseloads and visit schedules.
  • Implement medical plans of care, nursing interventions, and procedures to ensure safe, effective, quality care.
  • Maintain up-to-date skills in pain control, symptom management, IV use and maintenance, wound VAC, and complex wound care.
  • Delegate duties to LPNs within their scope of practice to ensure care plans are followed.
  • Evaluate and document patient responses and care outcomes to ensure quality of care.
  • Maintain accurate and timely nursing documentation in compliance with evidence-based practice and regulations.
  • Perform patient and caregiver education on medication side effects, treatment options, discharge instructions, and reportable conditions.
  • Complete required agency, state, and federal forms accurately and timely.
  • Conduct supervisory visits and communicate findings promptly.
  • Assess needs for Hospice Aide services, coordinate plans of care, and provide ongoing supervision.
  • Report patient incidents (e.g., infections, injuries) to Quality Improvement specialists as needed.
  • Coordinate safe patient discharges and transfers, ensuring proper documentation.
  • Collaborate with physicians, providers, team members, and vendors to support care plans.
  • Participate in monthly peer review audits and bi-weekly team meetings.
  • Obtain prior authorizations and communicate with insurance case managers regarding services and care plan changes.
  • Develop and deliver educational presentations internally and to community agencies.
  • Advocate proactively for patient care needs to ensure sensitivity to individual patient and family circumstances.

Required Qualifications

  • Valid Registered Nurse (RN) license in Vermont
  • Minimum of 2 years of hospice nursing experience, including hospice experience within the last year
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
