Contract Length

• 13 weeks

Role Summary

Provide safe, competent wound, ostomy, and continence nursing care for a designated patient group under the direction and supervision of the Wound and Ostomy Manager.

Key Responsibilities

• Conduct wound, ostomy, and continence-focused health assessments and physical assessments
• Develop care plans based on patient diagnosis and individual needs
• Recommend treatments and evaluate patient response to care
• Assess and document care for wounds, ostomies, continence issues, fistulas, complex wounds, and pressure injury risk
• Support standards of care development, revision, and evaluation
• Collaborate with the care team and make referrals as needed
• Educate patients and families on prevention of complications and self-management
• Participate in quality improvement activities
• Provide culturally competent education to patients, families, and staff
• Maintain confidentiality and data security
• Perform other assigned tasks

Required Qualifications

• Valid RN license in New York
• Associate of Science in Nursing / Associate Degree in Nursing
• Wound Care (3 years of experience)
• Wound Care experience within the last year
• Certified Wound Ostomy Nurse (CWON) or Certified Wound Care Nurse (CWCN) or Certified Wound Specialist (CWS) or Wound Care Certified (WCC)
