LVN (Hospice) - Local Contract in California

Job Summary

• Provide patient care under the direction of a registered nurse or physician within the scope of the Vocational Nurse Practice Act and regulatory standards.
• Assist in assessment, planning, implementation, and evaluation of patient care.
• Follow patient identification procedures and handle specimen collection.
• Implement and document prescribed interventions and discharge plans.
• Recognize and address ethical, legal, and medical dilemmas in nursing practice.
• Maintain a safe patient care environment and assist with department equipment care.
• Complete accurate charting and record keeping, including equipment and quality measures.
• Collaborate with healthcare team members to ensure timely, safe, and quality care.
• Ensure a safe, HIPAA-compliant environment for patients and staff.
• Perform other duties as assigned.

Contract Length

• 13 weeks

Work Schedule

• Monday to Friday
• 3:00 PM to 11:30 PM
• 8 hours per day, 40 hours per week

Required Qualifications

• Valid LPN/LVN license in California
• Minimum 1 year of Long Term Care experience within the last year
• Current BLS certification issued by the American Heart Association
• High school diploma or its equivalent
• Licensed driver with insured, reliable automobile
• Understanding of the needs of terminally ill patients
• Hospice experience
