Required Qualifications
- Valid Interventional Radiology Technologist license in Utah
- Interventional Radiology Technologist (1 year of experience)
- Interventional Radiology Technologist experience within the last year
- ACLS - Advanced Cardiac Life Support issued by American Heart Association
- BLS - Basic Life Support
- PALS - Pediatric Advanced Life Support issued by American Heart Association
- Government-issued photo ID
- E-Verify
- Annual skills checklist
- Annual competency exam
- 3 professional references, including at least 1 supervisor/manager or above
- Background check
- Drug screen
- Flu vaccination or approved exemption
- TB screening
- MMR immunity
- Varicella immunity
- Hepatitis B immunity
- Tdap within 10 years
- Physical exam within 1 year of start
- Respirator medical clearance if fit testing onsite is needed
- Orientation safety manual acknowledgement
- Interventional Radiology Tech document
