Physical Therapist - Home Health (Travel/Contract)

Location: Hayward, California
Contract Length: 13 weeks
Shift: Day shift, 9:00 AM – 5:00 PM (40 hours/week)
Weekend Requirement: As needed
On Call: No

Job Summary

Seeking a Physical Therapist for a home health travel contract position. The role involves providing therapy services in Alameda or Contra Costa County with a focus on patient care in home health settings.

Required Qualifications

• Valid Physical Therapist license in California
• Minimum 1 year of Physical Therapist experience within the last year
• Experience in a Home Health setting
• Experience with OASIS documentation
• Basic Life Support (BLS) certification

Certifications & Competencies

• BLS (Basic Life Support) certification from American Heart Association or American Red Cross
• Medication Test/Pharmacology Competency Exam (if applicable)
• Competency exams as required by profession, meeting or exceeding passing scores

Compliance & Documentation Requirements

• Government-issued photo ID (Driver’s License or State ID)
• Emergency Contact Form prior to start
• Background checks within 1 year of start, including SS Trace, County searches (7 years), Nationwide, and Sex Offender registry
• Annual EPLS/GSA/SAM and HHS/OIG checks
• Physical exam within 1 year of start
• Negative TB test (PPD, Quantiferon Gold, or T-spot) within 1 year; if past positive, negative chest X-ray within 1 year
• Immunization or titer proof for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B (titer, declination, or vaccine series)
  • Varicella (titer, vaccine, or history of chickenpox)
• 9 Panel Drug Screen within 1 year of start (instant drug screen acceptable)
• Seasonal flu vaccination or declination during flu season (Oct 1 – Mar 31)

References & Skills

• Two professional references required for travel, allied, or per diem acute care roles (within 3 years)
• Annual skills checklist required

Additional Information

• Training provided for Kinnser/Wellsky software systems used for documentation and scheduling
