Job Opportunity: Home Health Nurse (LVN / LPN)

Location: Manchester, New Hampshire
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Details

  • Schedule: 40 hours per week, Day shift (8:00 AM - 4:30 PM)
  • Weekend work may be required as needed
  • No call or float responsibilities
  • Care provided in patients' homes; candidate must own a vehicle
  • Average daily census: 415 patients
  • Patient age group: Adults and Geriatrics
  • Patient diagnoses include medical and surgical conditions such as wound care, ostomies, indwelling catheters, CVA, cancer, edema, sepsis, COPD, CHF
  • Special equipment used: CoaguChek, phlebotomy supplies, Aspira drains
  • Epic charting experience required
  • Skills checklist will be completed during onboarding

Required Qualifications

  • Valid Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license in New Hampshire
  • Minimum of 1 year of LPN/LVN nursing experience within the last year
  • Home Health nursing experience
  • Basic Life Support (BLS) certification issued by the American Heart Association
  • Phlebotomy skills
  • Experience with Epic electronic health record system

Background and Compliance Requirements

  • Comprehensive background check within 30 days of start, including:
    • Social Security verification
    • National sex offender registry search
    • National and state criminal searches (excluding NH counties; NH State Police search required)
    • Motor vehicle background check (must meet specific driving record criteria)
    • Education verification (high school diploma acceptable for CNAs)
  • Drug screening: 10-panel drug test within 14 days prior to start (no exceptions)
  • Two current professional references within the last 3 years, including one from a supervisor or charge nurse
  • Employment application and skills checklist required
  • Annual internal evaluation required for travelers with 1+ year tenure

Certifications and Training

  • Basic Life Support (BLS) - AHA provider only
  • Facility-specific orientation and safety manual acknowledgment (including OSHA education)
  • Pass/fail testing for Basic Dysrhythmia (EKG/Medication) may be required onsite depending on unit assignment
  • Crisis Prevention Institute (CPI) certification preferred
  • Management of Assaultive Behavior (MAB) certification accepted (online providers allowed)
  • NIH Stroke Certification valid for 2 years (if applicable)

Health and Immunization Requirements

  • Physical exam within 1 year prior to start (no expiration)
  • Annual fit mask test (N95 or equivalent) required; CAPR training available onsite if needed
  • Tetanus/Diphtheria/Tdap vaccine within last 10 years required; medical or religious exemptions allowed with documentation
  • Negative TB test or chest X-ray within 1 year prior to start; TB questionnaire required annually
  • Immunity or vaccination documentation required for:
    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Hepatitis B (declinations accepted with waiver)
    • Varicella (history of disease not accepted)
  • Seasonal flu vaccination required during October 31 - April 1 period; medical and religious exemptions allowed with documentation
  • Respiratory safety clearance questionnaire required prior to start

Additional Notes

  • Candidates who have ever been employed by any associated facility are not eligible
  • Proof of auto insurance required due to travel to patient homes
  • Facility-specific compliance attestations and waivers may be required (e.g., flu, Hep B, Tdap)
  • Passport-style photo required for badge issuance

This position offers an opportunity to provide specialized home health nursing care with a focus on adult and geriatric patients in a supportive travel contract environment.
