Licensed Practical Nurse (LPN) – Medical Surgical

Location: Rochester, New York
Employment Type: Travel/Contract
Contract Length: 13 weeks
Shift: Night (7:00 PM – 7:00 AM)
Hours per Week: 36 (3 shifts/week)
Weekend Requirement: Every other weekend
Holiday Requirement: Available to work every other holiday, if needed

Job Description

• Provide nursing care on a complex care medical-surgical unit with medicine, orthopedic overflow, and complex long-term patients.
• Patient assignment ratio: 1:5–6.
• Must be able to take own patient assignment with RN support.
• Required to float within skillset as needed.
• Charting experience with EPIC required.
• Scrub color: navy blue and white combinations.

Required Qualifications

• Valid LPN / LVN license in New York (NY is not a compact state).
• Minimum of 2 years LPN / LVN experience, with experience within the last year.
• Medical-Surgical nursing experience.
• Experience with Epic electronic health record system.
• Clinical skills including:

  • Central line care/management
  • PICC line management
  • High Flow Nasal Cannula (HFNC)
  • Trach care/management
  • Heparin protocols
  • Pacemaker management (temporary and permanent)
  • Use of Rapid Response/Code teams

Requirements & Compliance

• Government-issued photo ID required.
• 7-year criminal background check within 3 months of start (including felony, sex offender, education verification, SSN trace, and immigration eligibility).
• Monthly EPLS/GSA/SAM and HHS/OIG background checks.
• Foreign education equivalency evaluation required if applicable.
• Education verification from primary source required.
• Primary source verification of state license and NURSYS verification within 30 days of start.
• Various certifications may be required based on unit description (e.g., ACLS, PALS, NRP, TNCC, EFM).
• Orientation safety manual acknowledgement required.
• Competency exams and medication/pharmacology competency testing as required.
• Facility-specific documents and attestations must be completed.
• Facility parking pass form required during orientation.

Health & Safety Requirements

• Physical exam within 1 year of start, updated annually.
• Fit mask test within 1 year of start (specific mask types accepted).
• Negative TB test or chest X-ray within 1 year of start; TB questionnaire accepted in lieu of annual renewal.
• Immunizations or proof of immunity required for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Drug screen within 30 days of start (includes amphetamines, cocaine, fentanyl, heroin, opiates, etc.).
• Seasonal flu vaccination or signed declination required during flu season.
• Snellen eye exam within 1 year of start.
• COVID vaccination documentation preferred but not required.

Additional Information

• First-time travelers accepted.
• Call-off policy allows client to cancel up to three shifts during the contract without penalty.
• Requests for time off, block scheduling, and special scheduling must be submitted upon application.
• Candidates must complete health-related requirements and lab work within 5 days of signing the contract.
• Prior travel experience is not required for this position.
