Job Description

• Provide care to patients primarily over 30 years of age in their homes; rare instances of pediatric patients.
• Coverage includes inpatient, outpatient, and home health settings; pediatric care possible but not required.
• Work primarily with the Home Care Team, including Skilled Nurse and Physical Therapist.
• Make home visits independently, averaging 1-4 visits per week.
• Weekly team meetings via Zoom for case conferences on acute home care patients.
• Typical schedule: Monday to Friday, 8 AM to 5 PM (or 7 AM to 4 PM), no weekends. Schedule may be adjusted based on department needs.
• Floating: 4-8 hours per week in Home Health; remainder of time in inpatient/outpatient settings; rare hospice involvement.
• Facility is a critical access hospital with a 20-bed inpatient/outpatient department.
• Dress code: Professional casual.
• Orientation: One-on-one training with agency staff until comfortable with documentation; standard training checklist completed with OT, department director, and office staff.
• Computerized charting and EHR system (Cerner) used.

Required Qualifications

• Occupational Therapist (OT) license in Nebraska.
• BLS certification (American Heart Association).
• Minimum 2 years of experience preferred in the specialty.
• OASIS assessment experience is not required.

Additional Information

• Must be comfortable working independently during home visits.
• Vehicles available for travel or reimbursement provided for travel incurred in own vehicle.
