Floating: May be required to float between Belleville and Shiloh or within the facility

Return to Office Restrictions: 7 days within 13-week contract
Guaranteed Hours:
- 36 hours/week for 3 shifts of 12 hours
- 48 hours/week for 4 shifts of 12 hours

Required Certifications

ACLS (Advanced Cardiac Life Support) - AHA only
BLS (Basic Life Support) - AHA only
NIH Stroke Certification
Other certifications may be required depending on unit (refer to unit descriptions)

Licensure

Must have valid Illinois RN license (no pending licenses accepted)
Primary source verification required
NURSYS verification required

Background Checks & Compliance

Criminal background check within 6 months (7-year search including statewide and nationwide)
EPLS/GSA/SAM checks quarterly
HHS/OIG checks quarterly
Fingerprinting required for assignments at or floating to long-term care or rehab units in Illinois
Education verification of highest nursing degree required
State-specific exclusion and sanction list checks for Illinois
Additional state-specific checks for Missouri if applicable

Competency Requirements

Medication Test/Pharmacology Competency Exam (must meet passing score)
Unit-specific competency exam (updated annually)
Color blind testing (online)
Skills checklist within 1 year of start (oncology skills checklist if applicable)

Health & Immunization Requirements

Physical exam within 1 year of start
Fit mask testing (annual renewal required)
Tdap vaccine or declination (expires every 10 years)
Negative TB test or chest x-ray (2 negative PPDs or equivalent within 1 year)
Measles (Rubeola), Mumps, Rubella, Varicella immunity or vaccination/declination
Drug testing within 1 year of start (must include specific panels)
Seasonal flu vaccination or approved exemption required during flu season (09/30 - 04/01)
