Job Summary

Seeking a Physical Therapist specializing in Home Health therapy. The role involves managing a caseload primarily consisting of geriatric patients, with an average of 5-7 patients per day. This position requires flexibility in travel within a 60-mile radius from the office hub.

Required Qualifications

• Valid Physical Therapist license in Virginia
• Minimum 1 year of recent Physical Therapist experience
• Current BLS (Basic Life Support) certification

Background and Compliance Requirements

• Background checks including SSN trace, county searches, nationwide sex offender, motor vehicle records, education and work history verification, and state-specific searches as applicable
• Fingerprinting may be required depending on state placement
• Verification of highest degree related to profession
• Annual signed acknowledgment of Orientation Safety Manual
• Medication and competency exams as applicable
• Proof of current auto insurance required due to driving responsibilities
• Physical exam within last 12 months
• Negative TB test or chest X-ray as applicable
• Required immunizations or documented declinations: Measles, Mumps, Rubella, Hepatitis B, Varicella
• 9-panel drug screen within last 12 months
• Seasonal flu vaccination or facility-specific declination form during flu season
• COVID vaccination or facility-specific declination

Additional Requirements

• 2 professional references within 2 years of start date
• Full 7 years of work history documented with no gaps
• Annual skills checklist and work history verification within 90 days of start
• Completion of facility-specific policies and forms prior to start

• Candidates must be flexible with territory and able to work on-call weekends as scheduled.
