Overview
- Homecare RN role providing care for hospice/home health adult and pediatric patients in their home setting.
- Day shift, 10 hours per day, 40 hours per week.
- Contract length: 13 weeks.
Required Qualifications
- Valid RN license in Utah.
- Home Health experience within the last year.
- Home Health experience: 1 year.
- BLS certification for healthcare providers.
- Associate of Science in Nursing / Associate Degree in Nursing.
- Driver's License.
Preferred Qualifications
- Two years working as an RN.
- Previous experience in an Acute Care, Home Health, or Hospice setting.
Key Responsibilities
- Assess, plan, implement, and evaluate care for patients and families in the home setting.
- Work autonomously while coordinating care with interdisciplinary team members.
- Develop individualized plans of care and prepare patients for ongoing support needs.
- Direct the interdisciplinary care team through delegation, coordination, and collaboration.
- Evaluate patient response to interventions and revise the plan of care as needed.
- Electronically transfer patient files to the office at least twice daily.
