Required Certifications & Licenses

• BLS (Basic Life Support): Must be American Heart Association (AHA) certified
• Nursys Verification: Within 30 days prior to start; disciplinary actions require additional documentation
• Current Nursing License: Primary source verification within 30 days prior to start
• Ascension Color Vision Test: Completed prior to start and updated every 3 years
• Ascension Core Competencies: HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start and updated yearly (minimum 80% score, max 2 attempts)
• NIH Certification: Group A updated yearly, Groups B-F every 2 years
• Ascension EKG Exam: Must pass with at least 80%, max 2 attempts; updated as new editions are released
• MOAB Moderate Risk De-Escalation Training
• Ascension Cerner Test-Out: Cerner system competency testing required
• Fit Test: Required for all travelers; preferred prior to start, can be completed onsite; updated annually
• Driver’s License/State ID: Must be valid and match address requirements

Onboarding & Training Requirements

• Ascension Onboarding Requirements Education Attestation
• Ascension Nursing Contract Worker Onboarding: Completed onsite
• Ascension Nova StatStrip Training: Pre-hire modules for non-allied positions
• Ascension Skills Checklist: Clinical skills checklist completed within one year of start and updated annually
• Ascension 12 Panel Drug Screen: Completed within 30 days prior to start and updated annually; includes testing for 12 specified substances
• Ascension Badge Photo: Forward-facing headshot with white or blue background, no alterations

Health & Immunization Requirements

• MMR: Documented proof of two vaccines or positive IGG titer; no declinations accepted
• Hepatitis B: Documentation of vaccination series completion or signed declination
• Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement
• Positive PPD / Negative Chest X-Ray and Annual TB Questionnaire: Must provide positive IGRA or PPD results, chest x-ray within a year, and complete TB questionnaire annually
• 2 Step PPD OR IGRA (Quantiferon/Tspot): Required within 1 year and 30 days of start
• Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider; no declinations accepted
• Tdap: Valid within 10 years; no declinations accepted
• Health Statement: Statement of good health or physical exam prior to first assignment, updated annually

Professional Documentation & Verification

• Ascension Profile (Resume): Required for submission
• Professional References: At least two professional references prior to first assignment
• Criminal Background Check: Includes SSN trace, employment verification, OIG search, national sex offender check; updated annually or after breaks in service >90 days
• EPLS (Excluded Parties List System) Check: Within 90 days of start; rerun if break in service >90 days
• Education Verification: Must be completed within 90 days of start; official transcripts required if verification fails
• Employment Verification: Third-party verification for past 7 years; updated annually or after breaks in service >90 days
• Confirmation of Assignment: Provided upon initial assignment and updated with extensions

Optional / Additional Requirements

• Ascension Unit Specific Checklist: Optional for onboarding, must be uploaded within a week by unit leader
• Hiring Manager Evaluation: Post-assignment evaluation (optional)
• Name Change Documentation: Legal documentation if applicable (optional)
