Physical Therapist - Home Health (Travel/Contract)

Location: San Jose, California (starting in Salinas, may work out of Santa Cruz if no census)
Contract Length: 13 weeks

Job Details

Employment Type: Travel/Contract
Shift: Day shift, 8 hours per day (7:00 AM – 3:00 PM), 5 days a week
Guaranteed Hours: 36 hours weekly
Patient Load: 5-6 patients per day (1:1 ratio, may visit up to 6 patients daily)
Weekends: Required if needed
On-Call: Not required
Locals Accepted: Yes

Documentation: Timely communication and documentation required; documentation tools provided and must be returned
Transportation: Traveler must have their own transportation
Travel Radius: Occasional travel up to 50+ miles; system groups clients to minimize drive time

Mileage Reimbursement: IRS standard

Required Qualifications

Valid Physical Therapist license in California
Minimum 1 year of Physical Therapist experience within the last year
Experience in a Home Health setting
Experience with OASIS
BLS certification

Compliance process takes about 2 weeks before start date
