Patient Ratios: Typically 1:5 or 1:6, may fluctuate with census and call-ins
EMR System: Meditech
Scheduling: Every other weekend required; float within scope of practice required; blocked scheduling not accommodated
Residency Requirement: Must reside within 75 miles of the facility

Valid RN license in West Virginia
Minimum 2 years of Med Surg / Telemetry nursing experience, with at least 1 year of recent experience in Med Surg / Telemetry
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
American Heart Association Basic Life Support (BLS) certification
Advanced Cardiac Life Support (ACLS) certification
NIH Stroke Scale (NIHSS) certification

Annual physical exam or physician statement confirming clearance to work without restrictions
Fit mask test to be completed onsite (PAPR not accommodated if facial hair present)
Immunizations and documentation required prior to start:
- Tetanus/Diphtheria/Tdap (valid within 10 years; Td not accepted)
- Negative TB test (QuantiFERON blood test within 1 year; T-Spot/PPD not accepted)
- TB questionnaire if TB positive
- Proof of immunity or vaccination for Measles, Mumps, Rubella (2 MMR doses or positive titer)
- Hepatitis B series, titer, or declination (declination must be within 1 year)
- Varicella vaccination or positive titer (2 doses or proof of first dose to start)
- Seasonal flu vaccination or approved medical/religious exemption during flu season
Drug testing: 9-panel drug screen (no THC) with expanded opiates and oxycodone, completed within 30 days of start and annually updated

Background checks per Appalachian Regional Bundle with expanded opiates and oxycodone panel, including:
- National criminal search, SS & address trace, sex offender, fugitive, violent offenders, county searches (last 7 years)
- FACIS 3 or equivalent, SAM/EPLS, OIG, OMIG, OFAC verifications
- Education verification included
State-specific checks:
- West Virginia APS/CPS search (annual update required)
Employment application and skills checklist required annually
Facility-specific documents and signed job description required, renewed annually
Orientation occurs bi-weekly; start date is immediate upon assignment confirmation
