Certifications Required

• Required:

  • BCLS (Basic Cardiac Life Support)
  • NIH Stroke Certification (required for all nurses)
• Preferred:

  • ACLS (Advanced Cardiac Life Support)
  • CPI (Crisis Prevention Institute) or MAB (Management of Assaultive Behavior) if applicable

Special Requirements

• Telemetry skills including reading rhythms
• Experience with cardiac medications and drips (no titration)
• Adherence to dress code: no artificial or nail enhancements, no Crocs with holes, blue uniform

Compliance & Documentation Requirements

• Government-issued ID required
• Criminal background check within 6 months (7-year search including statewide, nationwide, and international if applicable)
• Quarterly EPLS/GSA/SAM and HHS/OIG checks
• Education verification of highest nursing degree required before start
• State-specific background checks and registries as applicable (Missouri and Illinois)
• Professional license primary source verification (NURSYS for Missouri licenses)
• Drug testing within 1 year of start, must include specific panels and be valid throughout assignment
• Physical exam within 1 year of start
• Fit mask testing (N95 required, must be renewed annually)
• Immunizations and health screenings:

  • Tdap vaccine or declination (expires every 10 years)
  • Negative TB test or chest x-ray (2 negative PPDs or equivalent)
  • Measles, Mumps, Rubella, Varicella immunity or vaccination/declination as per unit requirements
  • Seasonal flu vaccination or approved exemption required during flu season
• HIPAA Acknowledgement, Latex Allergy Questionnaire, Confidentiality Form, Hand Hygiene Policy, Fingernail Policy, Dress Code Policy to be reviewed and signed prior to start
• Facility-specific regulatory compliance education to be completed annually
• Badge photo required (head and shoulders, white background, no obstructions)

Competency Requirements

• Medication/Pharmacology competency exam (must meet passing score)
• Unit-specific competency exam updated annually (Oncology competency required if applicable)
• Skills checklist within 1 year of start (Oncology/Infusion skills if applicable)

References & History

• Two professional references required (supervisors preferred)

• Performance evaluations may be requested but not mandatory for extensions or per diem
