Required Credentials & Compliance

Certifications & Licenses

  • BLS (Basic Life Support): Must be American Heart Association (AHA) certified.
  • Nursys Verification: Must be completed within 30 days prior to start. Disciplinary actions require completion of FACIS Questionnaire.
  • License/Certificate Verification: Current license and primary source verification within 30 days prior to start. FACIS Questionnaire required if disciplinary actions exist.
  • NIH Certification: Group A updated yearly; Groups B-F updated every 2 years.
  • MOAB Moderate Risk De-Escalation Training: Required.
  • Driver's License/State ID: Must be valid, not expired, and address must match home state for compact licenses.

Testing & Checklists

  • Ascension Core Competencies: Includes HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start and updated yearly. Minimum 80% score required.
  • Ascension EKG Exam: Must be completed with a passing score of at least 80%. Only two attempts allowed.
  • Ascension Cerner Test-Out: Required Cerner test results and training checklist with signed attestation.
  • Ascension Skills Checklist: Clinical skills checklist to be completed within one year of start and updated annually.
  • Ascension 12 Panel Drug Screen: Must be completed within 30 days prior to start and updated annually. Includes testing for 12 specific drugs with proof required.
  • Ascension Unit Specific Checklist: Optional for onboarding but must be uploaded within a week if completed.

Personal Health History & Vaccinations

  • Ascension Color Vision: Completed prior to start and updated every 3 years.
  • MMR (Measles, Mumps, Rubella): Documented proof of two vaccines or positive IGG titer required; no declinations accepted.
  • Fit Test: Required for all travelers; preferred prior to start but can be completed onsite. Updated annually.
  • Hepatitis B: Documentation of vaccination series completion or signed declination required.
  • Influenza Vaccine: Required for flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement.
  • Positive PPD/Negative Chest X-Ray and Annual TB Questionnaire: Must provide positive IGRA or PPD results, chest x-ray within a year, and completed TB questionnaire. Annual updates required.
  • 2 Step PPD OR IGRA (Quantiferon/Tspot): Required within 30 days prior to start.
  • Varicella: Proof of two vaccines, serological immunity, or history verified by healthcare provider. No declinations accepted.
  • Tdap: Valid within 10 years; no declinations accepted.
  • Health Statement: Statement of good health or physical exam prior to first assignment, updated annually.

Professional & Employment Verification

  • Ascension Profile: Resume or professional profile required.
  • Ascension 2 Professional References: At least two professional references required prior to first assignment.
  • Ascension Criminal Background Check: Includes Social Security Trace, Employment Verification, OIG Search, National Sex Offender check; completed within 30 days prior to first assignment and updated annually.
  • Ascension EPLS (Excluded Parties List System): Must be run within 90 days prior to start and rerun if break in service > 90 days.
  • Ascension Education Verification: Verification of education relevant to clinical work within 90 days prior to start; must be rerun if break in service > 90 days.
  • Ascension Confirmation of Assignment: Must be provided upon initial assignment and updated with each extension.
  • Ascension Employment Verification: Third-party employment verification for past 7 years; updated annually or if break in service > 90 days.

Onboarding Requirements

  • Ascension Onboarding Requirements Education Attestation: 6-hour pre-hire education with attestation required.
  • Ascension Nursing Contract Worker Onboarding: Completed onsite prior to start.
  • Ascension Nova StatStrip: Pre-hire modules required for non-allied positions prior to clearance.

Optional Documents

  • Hiring Manager Evaluation: Post-assignment evaluation (optional).
  • Ascension Name Change Document: Legal documentation of name change (optional).

This position requires strict adherence to credentialing, health, and compliance standards prior to and during the assignment.
