Registered Nurse (RN) - Home Health

Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks


Job Summary

  • Provide patient care in home health settings including assessment of patient conditions, implementation of care plans, and ongoing education/support for patients and families.
  • Perform admission processes and case management duties following certified home health agency policies and standards.
  • Deliver care consistent with community health practice philosophy and organizational standards.
  • Weekend rotations occur approximately once every 6 weeks.
  • Ability to travel to assigned cases is required.

Required Qualifications

  • Valid RN license in Massachusetts.
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
  • Minimum 1 year of Home Health nursing experience within the last year.
  • Minimum 1 year of Case Management experience in a Home Health setting.

Required Credentials & Compliance (must be completed prior to or at onboarding)

  • Professional license verification and primary source validation.
  • Submission of a recent (within 6 months) professional photo meeting specified criteria.
  • Motor Vehicle Record (MVR) check within 45 days of start.
  • Criminal background check, including Massachusetts CORI and national sex offender registry, within 45 days of start.
  • Exclusion checks against OIG, GSA, MassHealth, and NH Medicaid lists.
  • Nurse Aide Registry check (required for all employees in Massachusetts).
  • Verification of highest education level.
  • Submission of a detailed resume covering the past 7 years.
  • Two professional references within the last 4 years (at least one supervisor-level).
  • Completion of a skills checklist within 1 year of submission.
  • Fit test for N95 respirator within 365 days of start (no religious or medical exemptions).
  • Respirator Medical Clearance attestation if fit test required.
  • Documentation of immunizations: MMR (measles, mumps, rubella) or proof of immunity.
  • TB screening documentation within 3 months of start (skin test or blood test) and updated if there is a break >6 months.
  • Health statement signed by a licensed provider within 365 days of start and updated if >6 month break.
  • Submission of a candidate submission coversheet for initial assignment.

Additional Information

  • Prior travel experience is not explicitly required.
  • Must be able to travel to patient homes as assigned.
  • Positive TB test requires documentation of a negative chest x-ray (optional but recommended).
  • Name change documentation required if legal name differs from credential documents.

  • Day shift: 08:00 AM – 04:30 PM
