Certifications:
- BLS (American Heart Association or Red Cross)
- ACLS (American Heart Association or Red Cross)
- DCI/BVP (Workplace Violence Prevention training)
Licenses & Professional Requirements:
- Valid professional license verification (Primary Source Verification within 30 days of start)
- Driver’s License, State ID, or Passport
- Prime OIG check
- Prime SAM/EPLS exclusion search
- Prime Criminal Background Check (within 1 year; some states require before start)
- Prime Agency Attestation (completed after all credentials uploaded)
- Prime Form I-9 (Employment eligibility verification)
- Prime Badge Photo (clear headshot, JPEG)
Testing & Exams:
- Prime Core Competencies Exam (minimum 80%)
- Prime Skills Checklist
- Prime Specialty Exam (minimum 80%)
- Prime Basic Pharmacology Exam (minimum 80%)
- Prime Basic Rhythm Interpretation Screening (minimum 80%)
- Annual Respirator Fit Test (must be completed prior to start if not available onsite)
- Prime Drug Screening (minimum 10-panel test including THC, within 60 days of start)
